What is the start-of-elementary-school health checkup?

The start-of-elementary-school health checkup is performed according to the School Health and Safety Act to ascertain the health of your child and provide health advice to help them prepare for elementary school.
This is a health checkup performed by the municipality for children who will enter elementary school (including the compulsory education school early course and the elementary department of special support schools) the following April.
The health checkup will include a physical examination, eye examination, and dental examination performed by the school doctor/dentist, and a vision test, interview, and, as needed, a hearing test and educational consultations will be performed by faculty.
The doctor will perform these tests and examinations to uncover diseases and other concerns early on that could affect school life or daily life and make recommendations for further examination.

Cancellation of the start-of-elementary-school health checkup

The 2024 start-of-elementary-school health checkup (for children planning to enter elementary school in April 2025) could be canceled on the day of the checkup due to typhoons or other natural disasters or the spread of infection.
Check the website for information on cancellations on the day of the checkup before coming.

The 2024 start-of-elementary-school health checkup (for children planning to enter elementary school in April 2025) will be performed by the school listed in the start-of-elementary-school notification according to the following schedule. The dates and times vary by school. Ensure that the you come at the correct date and time.

The checkup will be performed at the elementary school specified for addresses in the district.
You cannot get the checkup at a different location.

Weekday afternoons in the end of October through the beginning of December (the checkup dates and times vary by location)

Checkup guide

Eligible families (families registered in Yokohama City as of October 1) will be sent a start-of-elementary-school notification and guide to the start-of-elementary-school health checkup in the middle of October by the ward office. It will include the checkup location and date.

Notes on the checkups

Please do not come on the day of the checkupby car, motorbike, or bicycle.
The start-of-elementary-school health checkup usually takes about 1.5 to 2 hours. Please understand that the number of children being examined and the progress of the health checkups may cause the checkup to take longer than planned.
Contact the school or the Health Education and Food Education Division regarding whether the checkups will still be performed in cases of storm warnings due to typhoons, infectious diseases, etc. You can also check the website.

Request for guardians regarding the start-of-elementary-school health checkup

  • Please bring the start-of-elementary-school notification, the survey slip for the start-of-elementary-school health checkup, indoor footwear for you and your child, a bag for you and your child's outside footwear, a writing utensil, and a handkerchief. On the day of the health checkup, check the health of anyone coming to the school before coming.
  • Do not come for the start-of-elementary-school health checkup if anyone has a fever (37.5°C or higher).
  • If your child cannot be examined because you or your child has a fever (37.5 °C or higher) or because of other reasons, contact the elementary school where the health checkup is being performed. Please understand that the health checkup cannot be provided on a different day or at a different location. Missing the start-of-elementary-school health checkup does not have an effect on school admission. The health checkup will be performed right after admission to the elementary school, so it is generally unnecessary to have a separate checkup at a medical institution.

FAQ

Q. Can my child get the health checkup at a different school if the specified day doesn't fit my schedule?
A. Your child can only get the health checkup at the specified school. Please make your best effort to adjust your schedule so that you can go to the specified school. If you cannot go due to work or the health condition of your child, inform the school that your child will be absent. As the start of school approaches, we will provide you with a flier or a notification.


 
Q. My child will be unable to get the health checkup for scheduling reasons. Can they still enter elementary school?
A. Missing the health checkup does not have an effect on school admission. You are not required to get a separate health checkup at a medical institution.


 
Q. My child is planning to enter a private or national school, do they still need a health checkup?
A. Your child is still expected to get the health checkup in both cases. If your child is admitted to a private or national school after the checkup, inform the school that performed the health checkup. The health checkup slip issued after the checkup will be sent by the board of education to the school where your child plans to attend. If your child is admitted prior to the health checkup, the school may not require a start-of-elementary-school health checkup. Therefore, contact the school directly. If your child will not get the health checkup, inform the school performing the checkup that your child will be absent.


 
Q. I will not be able to come with my child for the health checkup due to scheduling reasons.
A. The parents do not have to come. Your child can be accompanied by their grandparents, etc. Have your child be accompanied by someone who understands your child's circumstances and health condition.


 
Q. Do I need my Mother and Child Handbook?
A. No.


 
Q. I lost the survey slip for the start-of-elementary-school health checkup that was enclosed with the start-of-elementary-school notification.
A. The school will have extras. Let them know that you need one. If you fill out the survey at the checkup location, be aware that you need to fill out your child's vaccination history, in which case you will need the Mother and Child Handbook.


 
Q. We are planning on moving out of the ward (or city) during the start-of-elementary-school health checkup period.
A. (if the health checkup is prior to your move)
Get the health checkup at the specified school. The health checkup slip issued after the checkup will be sent by the school (or board of education) to the school where your child plans to attend (or the local government of where you are moving).

Talk to the ward or local government of where you are moving. Also, inform the school performing the checkup that your child will be absent.

Get the health checkup at your current location. Your child will not need to get another checkup after moving.

If the school where your child plans to attend after moving is performing the health checkup, then contact the school. If the health checkup at that school has concluded, your child cannot get the health checkup. Missing the health checkup does not have an effect on school admission.
