About National Health Insurance premiums
How National Health Insurance premium amounts are decided
National Health Insurance premium amounts for a fiscal year (i.e. from April to March of the following year) are decided every June. The amount each household pays is determined according to how many people in the household are signed up for the National Health Insurance scheme and income from the previous year (from January to December). Note that people aged 40 years and over and under 65 years also have to make additional payments under the Long-Term Care Insurance System.
If you have a change of circumstances after your National Health Insurance premium has been decided, such as the number of people enrolled in National Health Insurance in your household changes, or your income changes, or if there are no longer any insured persons under the scheme, your National Health Insurance premium will be recalculated.
You will be sent a notification in the post informing you of your National Health Insurance premium.
How to pay your National Health Insurance premium
The National Health Insurance premium is spread over 10 monthly payments, from June until March. Payment methods include direct withdrawal from your bank account and other special arrangements (such as deduction from your pension).
Commencement of payment of National Health Insurance premium
You will start paying the National Health Insurance premium in monthly installments from the month that you became an insured person (left an insurance scheme that you were previously signed up for, not from the month in which you submitted notification). If you delay submitting notification, you will have to make back payments for all the months you missed while not signed up. Also, if you leave the National Health Insurance scheme, you will be required to make premium payments up until the month prior to which you left the scheme. Please make sure to submit notification as soon as possible.
If you fail to keep up with your National Health Insurance premium payments
1. You will be sent a reminder, demands for payment (prior notification that your assets will be seized), etc.
You are required to pay your National Health Insurance premium payment installments by the stipulated deadlines. If you fail to pay by these deadlines, you will be sent a letter of reminder as stipulated in the City's ordinances. You may also be sent other demands for payment (prior notification that your assets will be seized), etc.
2. Late payment charge
You will have to pay a late payment charge if you do not pay by the deadline specified in the reminder.
*Calculation of late payment charges
Late payment charges are calculated based on the rate of interest stipulated by ordinance for the number of days from the day after the deadline stipulated on the reminder until the date payment is made.

3. Proceedings against those who fail to pay the National Health Insurance premium (seizure of assets)
Your assets will be investigated in the event you fail to pay your National Health Insurance premium (National Tax Collection Act Article 141).
If the results of this investigation show that you have sufficient assets with which to pay off money owed on your National Health Insurance premium, legal proceedings may be initiated without warning to seize your assets, including real estate, savings, salary, life insurance, etc. (National Tax Collection Act Article 47).
4. For those who have permission to stay in Japan and fail to pay their National Health Insurance premium
Please note that if you are in arrears with your insurance premiums, you may not be allowed to renew your period of stay.
