Guide to the third COVID-19 vaccination shot(新型コロナウイルスワクチン3回目接種に関するご案内)

Vaccination Flow

  1. Reserve a vaccination slot
    Reservation applications can be made immediately with the enclosed vaccination voucher.
  2. Receive the vaccination
  3. The vaccination procedure is now complete
    Only one shot is required for the third vaccination.

Vaccinations

Either the Takeda/Moderna vaccine or Pfizer vaccine will be used for the third vaccination.

- The Takeda/Moderna vaccine will consist of half of the volume used for the first and second vaccinations.

It is possible to alternate between vaccines and receive a different vaccine to those received for the first and second shots.
Regardless of the vaccinations used on the first and second shots, you are requested to consider making a reservation to receive either of the vaccines available as early as possible.
* In comparison to the first and second vaccinations, there is a greater quantity of the Takeda/Moderna vaccine provided by the government for the third shot than the Pfizer vaccine. Due to this, it may be more difficult to make reservations for the Pfizer vaccine.

The government has announced the following:

- Full effectiveness and safety even when using a different vaccine to the first and second shots has been confirmed.
- People who received the Takeda/Moderna vaccine for the second vaccination and then received the Takeda/Moderna vaccine for the third (booster) shot experienced a greater chance of lymphadenia symptoms in comparison to the second dose, although fever, fatigue and other such symptoms were experienced to a lesser extent.

Reservations

It is necessary to obtain a reservation to receive a vaccination. Reservations can be applied for using the enclosed vaccination voucher. First we recommend that you check with your family doctor to see if you can receive a vaccination from them. The methods of applying for reservations are listed below. The vaccination venues differ depending on whether you reserve your vaccination with procedure (1) or procedure (2).

(1) Third Vaccination at Venues Reserved via the City

(1) Access the site

Use the above Link to access the dedicated reservation website.

※The dedicated reservation website does not support Internet Explorer.Use Microsoft Edge, Google Chrome, or another browser.

(2) Log in(People who used the site for reserving their first and second vaccination may proceed to (4).)

Input the "vaccination voucher number" from your vaccination voucher.

The password is your 8-digit (Western calendar) birth date. (E.g. January 1, 1960 → 19600101)

* See here if you have already set a password when reserving the first and second vaccinations.

(3) Register your email address, and check the confirmation email (first login only)
(4) Enter your name (in kana) and phone number
(5) Proceed to the reservation screen from [Reserve / Amend] from My Page
(6) Proceed to the search screen from [Select Vaccination Venue]
(7) Search for a vaccination venue

It is possible to run searches on [Vaccine Type], [Vaccination Venue] and [Date Specification].

(8) Select the date and time of the reservation
(9) Check to ascertain that the reservation has been completed, as well as the reservation details.
The confirmation screen will be displayed once the reservation has been completed.
A [Reservation Confirmation Notice] will also be sent to the registered e-mail address.

* The Dedicated phone for reservation cancellation only accepts the cancellation of (1) Third Vaccination at a Venue Reserved via the City. It is also possible to cancel reservations made on the reservation website.

(2) Medical institutions that accept reservations directly

The method of applying for vaccinations differs depending on the medical institution concerned. See the enclosed List of Vaccination Venues for further details.

It is now possible to run searches for vaccination venues with vacancies from the city website (both vaccination venues for which reservations are accepted by the city, and medical institutions that accept direct reservations). Searches can be run based on addresses, nearest stations and vaccine types, etc.
* The vacancies listed are correct at the time of the most recent update, and there are cases in which actual conditions are different.

Effects of the Third (Booster) Vaccination

After exhaustive clinical trials and research into immunity, etc., it has been reported that the third (booster) shot is effective in increasing the deteriorated efficacy of infection and serious illness prevention etc.
Third (booster) vaccinations are particularly recommended for the following people.

  • Elderly people, people with underlying illnesses and other people considered to be at high risk of serious infection
  • People interacting with those considered to be at high risk of serious infection, nursing care workers (nursing staff, etc.), and others who come into regular content with people considered to be at high risk of serious infection
  • Medical workers and other people at high risk of being exposed to the virus through occupational activities

Consultations Related to Side Effects

There are cases in which pain in the area of vaccination, fatigue, headaches, muscle and joint pain, chills, diarrhea, fever and other side effects may be experienced after vaccination. Most of these side effects will disappear within a few days of vaccination. Consult with your doctor or the medical institution at which you received your vaccination if these symptoms persist for longer periods.

Reservations by Proxy (For People Without PCs or Smartphones)

Eligible People

People who do not possess a computer, smartphone or other device, making internet (web) reservations difficult.

Details

The procedures for reserving by proxy, and cancelling or amending reservations for people wishing to receive a (1) Third Vaccination at Venues Reserved Via the City as listed in Reservations on page 1.

Hours

09:00 to 17:00 (weekdays only)
* Times may differ for certain post offices.
* Advance reservations are not required, although the counters may be crowded.

Third vaccination voucher

Ward Administration Offices(staff in each ward office designated for vaccination consultations)

In addition to accepting reservations by proxy for third vaccinations, these counters also provide consultations on various other topics, such as the first and second vaccinations, reservation support for medical institutions accepting direct reservations, vaccination certificates, etc.

Post offices in the city(counters manned by post office employees)

Note that these counters only accept reservations by proxy for third vaccinations, and do not provide consultations on any other subject.
Reservations by proxy will only be accepted at post offices until March 31st (Thursday), 2022.
Reservations by proxy are not accepted at the following four locations.

  • Kanagawa Post Office (Kanagawa Ward)
  • Tsubakigaoka Post Office (Kanazawa Ward)
  • Yokohama City University Hospital Post Office (Kanazawa Ward)
  • Yokohama Oroshihoncho Post Office (Seya Ward)

Points to Note on the Day of Vaccination

  • Take your temperature, and if it is clear that you have a fever or are otherwise feeling unwell, you are requested to forego the vaccination and cancel your reservation. (If you are due to receive your vaccination from a venue reserved via the city, use the special telephone number for cancelling reservations listed on page 1.)
  • Wear clothing to the venue that allows easy access to your shoulders.

Items to Bring on the Day

Things included in this envelope

  • Third vaccination voucher
    Do not remove the stickers.
    The bottom right portion of the vaccination voucher (vaccination certificate) is an important document certifying that the vaccination was performed. Keep the voucher after the shot. Do not dispose of it.
  • Pre-vaccination questionnaire
    Read the enclosed [Explanation on COVID-19 Vaccination] thoroughly, fill in the form in advance and take it with you to the vaccination venue.

Things to prepare

  • Personal ID (Individual Number Card, Health Insurance Card, etc.)
  • Medication handbook (if available)

Precautions

  • The details listed herein are correct as of January 14th, 2022. There may be cases in which the contents change due to revisions to governmental policies, etc. Check the most up-to-date information on the Yokohama website.
  • COVID-19 vaccinations are to be received under your own responsibility having fully comprehended both vaccination efficacy and the risk of side effects. You are also requested to refrain from forcing work colleagues and other people around you to be vaccinated, and from discriminating against anyone who is not vaccinated.
