This notification is being sent to individuals who are eligible to receive the 3rd~7th vaccination.
Vaccination will be from September 20 to March 31.
Vaccination Flow
1. Making Reservations for Vaccinations
Reservation applications can be made with the enclosed vaccination voucher.
2. Getting Vaccinations
As for children under the age of 16,guardians must accompany their children. (If this is difficult, a relative, etc., who is familiar with the health condition of the child to be vaccinated can be present on behalf of the guardian. A letter of attorney is not required, but the guardian's signature is required on the pre-vaccination medical questionnaire.)
3. Vaccination is Complete

- People 12 years old and above who have received the first and second vaccination shots can receive the vaccination shot for the omicron variant after 3 months have elapsed since receiving the previous vaccination shot.
- People 12 years old and above who have received the first and second vaccination shots can receive the conventional vaccination shot (Takeda Novavax) after 6 months have elapsed since receiving the previous vaccination shot.
- Even if you get a vaccination shot with a vaccine that is different from the previous vaccination, the efficacy and safety have been confirmed to be sufficient.
Vaccination interval between the COVID-19 vaccine and influenza vaccines or other vaccines
- You can get the COVID-19 vaccine and an influenza vaccine at the same time without needing time in between them.
- You cannot get vaccines other than the influenza vaccine at the same time as the COVID-19 vaccine. There must be at least 2 weeks between the COVID-19 vaccine and any other vaccine.
Reservations
It is necessary to obtain a reservation to receive a vaccination. Reservations can be applied for using the enclosed vaccination voucher. First we recommend that you check with your family doctor to see if you can receive a vaccination from them. The methods of applying for reservations are listed below. The vaccination venues differ depending on whether you reserve your vaccination with procedure A or procedure B.

A. Making reservations at vaccination venues where reservations are accepted by the City (medical institutions other than those listed in B.)

(1) Access the site
Use the above Link to access the dedicated reservation website.
(2) Log in(People who used the site for reserving their first and second vaccination may proceed to (5).)
Input the "vaccination voucher number" from your vaccination voucher.
The password is your 8-digit (Western calendar) birth date. (E.g. January 1, 1960 → 19600101)
* See here if you have already set a password when reserving the previous vaccinations.
(3) Register your email address, and check the confirmation email (first login only)
(4) Enter your name (in kana) and phone number
(5) Proceed to the reservation screen from [Reserve / Amend] from My Page
(6) Proceed to the search screen from [Select Vaccination Venue]
(7) Search for a vaccination venue
It is possible to run searches on [Vaccine Type], [Vaccination Venue] and [Date Specification].
(8) Select the date and time of the reservation
(9) Check to ascertain that the reservation has been completed, as well as the reservation details.
The confirmation screen will be displayed once the reservation has been completed.
A [Reservation Confirmation Notice] will also be sent to the registered e-mail address.

*The Dedicated phone for reservation cancellation only accepts the cancellation of the vaccination at a Venue Reserved via the City. It is also possible to cancel reservations made on the reservation website.
B. Making reservations at medical institutions that accept direct reservations (medical institutions other than those listed in A)
The method of applying for vaccinations differs depending on the medical institution concerned. See the enclosed List of Vaccination Venues for further details.

It is now possible to run searches for vaccination venues with vacancies from the city website (both vaccination venues for which reservations are accepted by the city, and medical institutions that accept direct reservations).
● You can search for a vaccination venue by address, nearest station, vaccine type, etc.
● Information on openings is updated periodically and may not be up to date.
● You cannot make an appointment directly from the search page for openings.
● For vaccination venues offering the Takeda (Novavax) vaccine, search by vaccine type and select “Other.”
Consultations Related to Side Effects
- The main side effects are pain at the injection site, headaches, muscle and joint pain, fatigue, chills, fever, etc. In rare cases, serious side effects such as shock or anaphylaxis could occur. If you have symptoms of concern after the shot, talk to the vaccinating physician or your family doctor.
- In extremely rare cases there are reports of possible myocarditis or pericarditis following vaccination. If you have symptoms such as chest pain, palpitations, shortness of breath, or swelling a few days after the shot, get examined at a medical institution immediately.
- In extremely rare cases, Guillain-Barre syndrome has been reported to occur after mRNA vaccinations (Pfizer, Moderna). If you have symptoms such as weakened limbs or numbness after the shot, get examined at a medical institution immediately.

Eligible People
People who do not possess a computer, smartphone or other device, making internet (web) reservations difficult.

The procedures for reserving by proxy, and cancelling or amending reservations for people wishing to receive a vaccination at "A. vaccination venues where reservations are accepted by the City".

* Advance reservations are not required, although the counters may be crowded.

Reservations by proxy are not accepted at the following four locations.

Points to Note on the Day of Vaccination
- Take your temperature, and if it is clear that you have a fever or are otherwise feeling unwell, you are requested to forego the vaccination and cancel your reservation. (If you are due to receive your vaccination from a venue reserved via the city, use the special telephone number for cancelling reservations)
- Wear clothing to the venue that allows easy access to your shoulders.

The bottom right portion of the vaccination voucher (vaccination certificate) is an important document certifying that the vaccination was performed. Keep the voucher after the shot. Do not dispose of it. - Pre-vaccination questionnaire
Read the enclosed [Explanation on COVID-19 Vaccination] thoroughly, fill in the form in advance and take it with you to the vaccination venue.
