- Vaccination vouchers (For children) are being sent to children ages 5 to 11 who are subject to the third,fourth,fifth vaccination.
- Guardians should check the following information.
- This vaccination is not mandatory. Understand both the preventive benefits and risk of side effects before considering whether the child should get the vaccination.
- In Yokohama City, Omicron strain adapted bivalent vaccine is available after March 20, 2023.
- Please see the link below for requirements for those eligible for vaccination after May 8, 2023. Even if you are not eligible, please keep your vaccination voucher until this Autumn.
1. Course of vaccination
- The vaccination voucher arrives
- Make a reservation for the vaccination → 2. Making a reservation
- Reservation applications can be made immediately with the enclosed vaccination voucher.
- Receive the vaccination→ 5.Points to Note and Items to Submit on the Day of Vaccination
※Covid-19 vaccine and influenza vaccine can be recieve at the same time.However ,The COVID-19 vaccination generally cannot be provided at the same time as other vaccinations. If the child will get the COVID-19 vaccination and a different vaccination, at least 2 weeks must pass between the vaccinations. (Exceptions: tetanus vaccine for cuts, emergency vaccines, etc.)
2. Making a reservation
The medical institutions providing the vaccination and the reservation procedure are explained on the Yokohama City website. Talk to your family doctor, and then make a direct reservation with one of the medical institutions on the list.

(1) Access the site
Use the above Link to access the dedicated reservation website.
*The dedicated reservation website does not support Internet Explorer. Use Microsoft Edge, Google Chrome, or another browser.
(2) Log in (People who used the site for reserving their first or second vaccination may proceed to (5).)
Enter the [Vaccination Voucher Number] listed on your vaccination voucher.
Your password when logging in for the first time is your own birthday in eight characters (YYYYMMDD)
(Example: If you were born on January 1st, 2015→20150101)
*If you have a password that you used previous vaccinations shots, please use that.
(3) Register your e-mail address and confirm receipt of e-mail (first login only)
(4) Register your name (Katakana) and telephone number (If not registered previously)
(5) Proceed to the reservation screen from [Reserve/Amend] on “My Page”
(6) Proceed to the search screen from [Select Vaccination Venue]
(7) Search for a vaccination venue
It is possible to search by categories such as [Vaccine Type], [Vaccination Venue] and [Specified Date] etc.
(8) Select the date and time of the reservation
(9) Confirm that the reservation has been completed and double check the reservation details.
A confirmation screen will be displayed, and an email will be sent to the email address registered.

3. Vaccines for children
- Pfizer makes a vaccine for children.
- The vaccine for individuals ages 12 and up is a different type of vaccine with a different dosage and administration.

Regarding children of age 11(after reaching age of 12)
Children should receive the adult vaccine if turn 12 years old.
Even if children turn 12 years old, they can use same vaccination voucher.
(Adult vaccine is available from the day prior to the 12th birthday.)
After May 8, 2023, only those with underlying medical conditions may receive the vaccine for ages 12 and older. Those who wish to be vaccinated must be vaccinated by May 7, or wait until the autumn of 2023.
4.Persons eligible for vaccination after May 8, 2023(5-11 years old)
Vaccination is available for those who meet any of the following criteria.
①Those who have not received their third vaccination
Those who have not been vaccinated for the third time can receive the Omicron strain adapted bivalent vaccine.
②Those who have received the third vaccination and have not received Omicron strain adapted bivalent vaccine
Those who have never been vaccinated against Omicron strain adapted bivalent vaccine can also be vaccinated.
③Those who have received the third vaccination and have one of the following underlying diseases
Persons with any of the following underlying diseases are eligible for vaccination, even if they have received one dose of Omicron strain adapted bivalent vaccine prior to May 7, 2023.
- Chronic respiratory diseases
- Chronic heart disease
- Chronic renal disease
- Neurological and neuromuscular disorders
- Blood disorders
- Diabetes and metabolic disorders
- Malignant tumors
- Rheumatoid arthritis, collagen diseases
- Endocrine disorders
- Digestive disorders, liver diseases, etc.
- Suppressed immune system due to congenital immunodeficiency disorders, HIV infection, or other diseases or treatments
- Other pediatric diseases, etc.
5. Points to Note and Items to Submit on the Day of Vaccination
- The vaccination is free.
- Guardians must accompany their children. (If this is difficult, a relative, etc., who is familiar with the health condition of the child to be vaccinated can be present on behalf of the guardian. A letter of attorney is not required, but the guardian's signature is required on the pre-vaccination medical questionnaire.)
- If the child to be vaccinated or the accompanying guardian has a fever or is otherwise unwell, cancel the reservation and change it to a different day.
- Have the child wear clothing that provides easy access to the shoulders.

• Do not remove the stickers.
• The bottom right portion of the vaccination voucher (vaccination certificate) is an important document certifying that the vaccination was performed. Keep the voucher after the second shot. Do not dispose of it.

• In the field for the telephone number, provide a number that can be used to contact a guardian during the day.
• A guardian must provide their signature in the corresponding field.
• Fill in the required items before bringing the sheet.

6. Consultation window on side effects
After the shot, the child may have pain at the injection site, headaches, diarrhea, muscle or joint pain, fatigue, chills, fever, swollen lymph nodes in the armpits,etc. The child will generally recover from these symptoms within a few days after the shot. If systems persist or are severe, talk to your family doctor or the medical institution that administered the shot.
In extremely rare cases there are reports of possible myocarditis or pericarditis following vaccination. If you have symptoms such as chest pain, palpitations, shortness of breath, or swelling a few days after the shot, get examined at a medical institution immediately.
In extremely rare cases, Guillain-Barre syndrome has been reported to occur after mRNA vaccinations (Pfizer). If you have symptoms such as weakened limbs or numbness after the shot, get examined at a medical institution immediately.
