COVID-19 vaccinations Notification from the City of Yokohama(新型コロナウイルスワクチン接種 横浜市からのお知らせ)

Check the individual notification that was sent with the "COVID-19 Vaccination Voucher" sent in June or July.

About this notification

This is a notification about the details of vaccine reservations for individuals not eligible for priority vaccinations.
• This information is current as of July 16, 2021.
• It is being sent to individuals who will be 12 to 64 years old in 2021.
• It is being sent to individuals who are registered as residents of Yokohama as of July 14, 2021.
• It is also being sent to individuals who have been vaccinated.
• The "Application for the Issuance of a Vaccination Voucher for Move-in Residents" is being sent to individuals within two weeks of completion of move-in procedures by those who have recently moved to Yokohama. If you want to be vaccinated in Yokohama, submit the application by mail.

Vaccination Reservations

Based on the national supply of the vaccines, there may be changes or interruptions.

Reservation method (1) Individual vaccinations (city medical institutions)

About 1,500 medical institutions are accepting reservations. There are about 1,000 medical institutions performing vaccinations for non-regular patients. See the city's website for details.

Reservation method (2) Mass vaccinations, large-scale vaccinations

Medical institutions that accept reservations for mass vaccinations, large-scale vaccinations, or reservations from the City can accept reservations online or by phone (*). It is easier to make a reservation online. For details, check the guide included with the "COVID-19 Vaccination Voucher" or the City's website.
*Individuals with a hearing impairment can make a reservation by fax.

Based on the supply of the vaccine, reservations will begin to be accepted in three stages, separated by age.

*Your age is defined as the age you become in 2021.
*After August 24, new reservations are planned to be accepted every Monday for mass vaccination and large-scale vaccination venues.

City of Yokohama's vaccination process

The City of Yokohama is implementing vaccinations in three ways. For each vaccination method, the venue, vaccine type, and scale of the vaccinations varies. Check the details when making your reservation.

Notes for your vaccination day

• The vaccination is free.
• Please fill out your pre-vaccination medical questionnaire at home.
• Take your temperature, and if you have an obvious fever or if you feel unwell, refrain from getting vaccinated, and cancel your reservation.
• Wear clothing to the venue that allows easy access to your shoulders.

Filling in the pre-vaccination medical questionnaire
• A guardian must print their name in the corresponding field.
• In the field for the telephone number, provide an emergency contact number that can be used to contact a guardian.

What to bring on your vaccination day

• Vaccination voucher (Do not remove the stickers.)
• Completed pre-vaccination medical questionnaire
• Personal identification (My Number card, health insurance card, etc.)
• Prescription Record (if you have one)
