Reservations for residents with hearing loss

People with hearing loss who cannot make a reservation on a computer or smartphone may apply for a reservation via fax.

Application period

Booster shot (third shot)

Booster shots (third shots) will be provided at some mass vaccination center and medical facilities.
The starting dates for reservations are listed below.
Please apply at up to ten days before the day you wish to get vaccinated.

*Currently, only people 18 and older can receive the booster shot (third shot). Booster shots (third shots) are not available for people between the ages of 12-17.

*People under the age of 16 are required to bring a guardian to the city vaccination venues (mass vaccination venues). Please note that you will not be able to receive a vaccination without a guardian. For details, see the page on vaccinations for people under 16.(machine translation)(外部サイト).

Reservation status

It will take a few days for us to contact you with the results of your application. Please wait for us to contact you.
Furthermore, we will contact you via fax if one of the following occurs:
1. Information is missing from your application.
2. All of the slots on your requested dates are filled.
3. If you do not have hearing loss.
*In the case of 1 and 2, you may resubmit your application.

Regarding application results

If you do not receive a result for your application after 5 days, there is a chance that a fax did not go through. Check the status of your application, and then send your fax number, vaccination number, and a message stating that you have not received your results via fax to 045-550-4226.

If you do not contact us, your reservation will be cancelled.

・Fax number: results will be sent to this number
・Name (katakana )
・Vaccination voucher number: the ten digit number on the top right corner of your voucher.
・Date of birth
・Date of your second shot
・“Third vaccination”
Your preferred dates and venues: check the venues on the application form, and fill in your top three choices.

Preferred time period: AM or PM
・“Second vaccination”
If you would like to be vaccinated at a mass vaccination center, circle
1. Reservation at same venue/same time* for four weeks after your first vaccination.
2. Reservation at different venue/different time
For 2, fill in your requested dates and venues.

Application results

・ We will respond via fax stating whether it was possible to make a reservation for you.
・There are some cases where we may not be able to make a reservation because the slots are filled.
・Depending on the application status, responses may be delayed.
・Fax reservations are for people with hearing loss that cannot make a reservation on a computer or smartphone. We will not respond to people who have already made a reservation via a method other than fax.

Mass vaccination centers

・Aides or sign language interpreters may come to the venue with you.
・You may also communicate with writing implements or a medical interpretation tablet.
・Using a tablet, we can translate voice to text or video call a sign language interpreter.
・The venue staff can offer assistance at the venue or for using the tablet if necessary.
・We can also lend out wheelchairs.
