COVID-19 vaccination certificates

Residents with an Individual Number Card (“My Number Card”) can be issued a digital version of the vaccination certificate for COVID-19. Those who have difficulties getting a digital version issued can apply for a paper version. Note that it is the decision of each individual whether or not to get the vaccine, and the issuing of official certificates does not make getting the vaccine mandatory.
1. Digital vaccination certificate *starting December 20, 2021
(1) Overview
Residents can be issued a digital version of the vaccination certificate using the official app from the national government and their Individual Number Card (“My Number Card”).
(2) Advantages of the digital version
•Certificates are issued at no cost.
◦There is no postage/shipping fee. However, applicants are responsible for any additional costs related to internet usage.
•Certificates can be issued immediately.
◦Limited to cases where the vaccination was recorded in the Vaccination Record System (VRS).
•Received certificates can be displayed at any time by opening the app.
◦After receiving the certificate, it can also be displayed in offline mode.
(3) What you will need
1. Smartphone (running either iOS13.7 or later, or Android OS 8.0, or later with NFC Type B support)
2. Individual Number Card (“My Number Card”)
3. *Personal identification number (PIN) (related to #2)
4. (Only if traveling overseas) Passport

■ About the personal identification number
•The personal identification number (PIN) is a series of 4 -digits that you set when you received your Individual Number Card (“My Number Card”).
•For some users, this 4-digit PIN may be different than the PIN used to print copies of your Certificate of Residence or other documents at convenience stores.
•If you mistakenly enter the wrong password 3 times, a lock will be placed on the card. If you have forgotten the password or if a lock has been placed on the card, visit the residence registration division of the Ward Administration Office for the ward where you live to request it to be reset.
•The service counters at the Ward Administration Offices are expected to be crowded when the offices open in the new year, so residents not in a rush to receive the certificate are encouraged to consider visiting the office at a later time.

•It will usually takes about 1 month after application before the Individual Number Card is ready to be issued. Please be aware when planning to apply and receive it.

(5) Important note
You may not be eligible for a digital vaccination certificate if your vaccination data was not recorded in the Vaccination Record System (VRS). If you cannot apply for this reason, consider applying for the paper version instead.
